Accidents are the unpleasant events that occur in the organization which enables the employer to learn from the accidents and enhance the safety solutions in the workplace through analyzing the root cause of the event and preventing accidents with the right lesson. 

Investigation should be done for the accident as they are unbiased to identify the exact root causes of the incidents and the instant question will be asked within the organization. The real condition for the mistake happened can be rectified

Accidents are investigated to know what about near misses? Based on the incident in the organization, a significant injury or death requires an investigation.

  • Was supervision done at the time of work?
  • Were employees wearing PPE properly?
  • Were employees trained about proper usage of PPE?
  • Was the worker wearing damaged PPE at the time of the incident?
  • Did PPE get damaged at the time of the incident?
  • Were employees trained to handle dangerous situations?
  • Was any distraction done while performing work to the worker physically or mentally?
  • Were any preventative measures taken to control the risk?
  • Was there any deviation in the normal procedure?
  • Did control measures performed properly or not?
  • Were the proper tools and equipment used to perform the concerned job?
  • Was the job performance more complex to handle than normal?
  • Was there any different chemical used or changes made to perform the job?
  • Did the employees use required PPE as per the job performed?
  • Did the supervisor conduct regular safety meeting to employees?
  • Was there were any equipment failure at that time safety alerts properly functioned or not?
  • Were employees was properly trained while performing work?
  • Were employees proficient before performing work?
  • Were they able to identify potential hazard?

These are the incident or accident questions  laid down by OSHA to ensure safety culture in the organization and to determine root causes of all the incidents.
